Welcome!  The health and safety of everyone attending the International Assembly in Orlando, Florida is of utmost importance, and we need all attendees to do their part in following applicable safety protocols.

To attend the International Assembly, you need to submit two requirements to attend:  Proof of vaccine AND negative COVID-19 test within 72 hours of the first day of the official program (or proof of recovery).  You should have already submitted your vaccination information.

Please enter your information and submit proof of your negative COVID-19 test within 72 hours of the first day of the official program (or proof of recovery).

Antigen, PCR and Nucleic Acid Amplification Tests (NAAT) are all acceptable and must be administered by a medical professional or certified testing agency.

Proof of recovery must be within 90 days of the first day of the official program and includes both the original positive test result (including patient name and test date and the name of the medical professional or lab that analyzed the results) and a letter from a licensed healthcare provider or public health official stating that the patient is cleared to travel.

Once submitted, the Vital Circle team will review, verify your submission and ensure it matches our policies.  After you complete both requirements, you’ll receive an email confirmation prior to the event. Please be prepared to show the email confirmation upon arrival to the International Assembly.

Your privacy is important to Rotary International and The Rotary Foundation (collectively, “Rotary”) and the personal data collected on this form is subject to Rotary’s Privacy Policy.

By agreeing to provide personal data about your COVID-19 vaccination, you understand this information will be used solely to facilitate your attendance to the International Assembly event.  This information may be shared with third party suppliers to support the health and safety protocols needed.  This information will only be stored until January 14, 2023, unless requested by Rotary to be deleted earlier.

  • Note: Please take your COVID-19 test within the time frame required for your event.
  • Note: Please ensure submitted images are clear and legible. If the image is not legible, it will not be verified. Images must be a “still” photo. “Live” photos are not accepted. If you are submitting an image of test results, the image must include your name, date of test taken, time of test taken, and time of test resulted.
